Why it comes first
A licensed facility goes through state plan review before construction, is built to healthcare-specific standards, and carries inspection and commissioning requirements that add real months.
An unlicensed medical office is a commercial tenant buildout with clinical requirements layered on. The difference is not a percentage. It is a different project with a different budget and a different timeline.
Where the assumption gets made
Almost always early, informally, and by somebody without the standing to make it.
- A broker comparing the space to a similar medical tenancy.
- An owner reasoning from a project they did in another state.
- A designer assuming the practice will not add procedures later.
- A contractor pricing what they were shown rather than asking.
What to do instead
Ask your design professional or your healthcare attorney, in writing, before you sign a lease. What license will this facility hold, does state plan review apply, and what is a realistic duration for it.
It is a short email. On a healthcare project it is the highest-value thing anybody does, and it is skipped constantly.
